How to Refer

To refer a client, please download and complete our referral form and email it to admin@chcare.com.au.

What to include

  • Patient name, date of birth and address.
  • Referrer name, provider number and practice details.
  • Date of referral and presenting concerns or symptoms.
  • Requested testing, such as tympanometry, audiometry, OAE, ABR, or paediatric/play audiometry.
  • Relevant medical history, accessibility needs and requested number of sessions, if applicable.

GP and Specialist Referrals:
Valid for 12 months unless otherwise specified by the referrer. This applies to both adults and children.

Paediatric Clients (infants through to 18 years):
A paediatric referral may cover multiple visits within a 12-month period. If ongoing care is required beyond this timeframe, a new referral will be needed.

Private Clients:
Referrals outside Medicare or funded program pathways are recognised for 12 months per referral.

Funded Programs:
Hearing Services Program, DVA, WorkCover and NDIS referrals follow their own program-specific requirements.

Common Reasons to Refer

  • Suspected hearing loss, speech or listening concerns.
  • Tinnitus, earwax build-up, dizziness or balance concerns.
  • Paediatric hearing concerns, school readiness, speech delay or newborn follow-up.
  • Diagnostic assessment required for further medical review or eligibility pathways.

Red Flags — Same-Day Action

  • Sudden sensorineural hearing loss.
  • Acute unilateral hearing loss or severe asymmetry.
